Suicide Awareness Survey- Your Feedback is Wanted

Dear Community Partner,

The St. Cloud VA Suicide Prevention team is requesting your participation and help with the second iteration of our community awareness questionnaire pertaining to community awareness and perceptions surrounding suicide. Every six months, we will be collecting community awareness questionnaires in our communities to measure changes in awareness and stigma related to suicide over time.

Some of you may have completed the first round of this survey in April 2019, but we invite you to complete this survey again and also disseminate the survey to your personal networks for us to capture as much information as we can on the perceptions of this issue in our communities. The survey will be open until October 31, 2019 and results will be shared after afterwards in an aggregate report of surrounding St. Cloud area. Please find the survey link below:

The survey takes approximately 10 minutes to complete and is completely anonymous.

This questionnaire is validated, which means it is shown to be a reliable means of gathering information. The results of this questionnaire will help shape Central Minnesota’s approach toward the Vision of reducing deaths by suicide to zero. Therefore, answering these questions is an important and meaningful way of getting involved. After all, preventing suicide is everyone's business.
