CAMHI Needs Assessment Survey

We are asking residents and providers in Benton, Sherburne, Stearns and Wright County to take our Needs Assessment survey (links are below)- estimated time to take it is 3 minutes.

The purpose of this survey is for CAMHI to obtain feedback from residents and providers in order to gauge what service(s) are most needed in our region, to identify gaps in services, help drive budget decisions, and potentially identify new and innovative ideas to improve services and best serve our community.

We recognize that many people may fit into both categories of resident and provider and therefore both surveys would be applicable. Should that be the case, please take both surveys! If you take both, we simply ask that you answer accordingly and from the perspective that the survey is designed for- residents answering as residents and providers answering as providers.

We are asking that everyone help us spread awareness on these surveys! Providers, PLEASE share these with your colleagues and those you supervise. Please ask each of them to do the survey with every client they are working with. The more responses we get from people, the better and more accurate the data will be. So please help us to better help our community!

The survey links have been EXTENDED and will now remain active until the end of May.
